School & Sports Physicals

School and sports physicals available now through August 31, 2026, at your local Family Medicine Clinic or Pediatrics.

SCHOOL PHYSICALS $75*

Kindergarten through 7th Grade

*Fee is paid at the time of visit and will not be sent to insurance. Lab and immunizations are not included. Students must provide a current immunization card. Please bring glasses or contacts to physical. If requesting to send to insurance, visit will be billed as Preventative Medicine. Please check with your insurance provider for coverage.

SPORTS PHYSICALS $35*

7th through 12th Grade

*No insurance claims will be filed. Students must provide a completed and signed school physical form. Payment due at the time of physical. Please bring glasses or contacts to physical. Due to Scoliosis check, no sports bras, please.
