Community Health Needs Assessment/Community Health Implementation Plan

Our Community Health Needs Assessment/Community Health Implementation Plan (CHIP) identifies health and medical needs specific to our service area and provides a plan to indicate how Faith Health will respond to such needs.

This document suggests areas where other area organizations and agencies might collaborate with us to achieve desired improvements for the health of our region. Faith Health is committed to meeting our obligations to efficiently deliver medical services. All not-for-profit hospitals are required to develop this report in compliance with the Affordable Care Act.
